We have created a page for you to get information and training resources to help you onboard and get started with using the EPSDT 8015/8016 ePDF for digital submission and using the EPSDT portal.
 

Updates

Since going live January 1, 2022, EPSDT Online has had several corrections and improvements added. Click here for release notes of changes you may have noticed.

Accessing the EPSDT portal

You can login to the EPSDT portal using the following address:

https://medquest.hawaii.gov/partners/epsdt

Go to the Forms and Training tab to get information on how to login.


 

Access the NEW fillable EPSDT 8015/8016 fillable ePDF. Click on the PDF icon to download the fillable ePDF. --------------------------------------------->

You will need the Adobe Acrobat Reader or Adobe Acrobat to open and complete the form.  To access Adobe Acrobat, click here.

Once you download the file, you will need to open the file in your "Open with system viewer" located on the bottom left of your browser.
