COVID-19 Vaccine for CCFFH & E-ARCH

The Moderna COVID-19 vaccine will be available beginning in January for both Medicaid members and caregivers in a community care foster family home (CCFFH) or an expanded adult residential care home (E-ARCH).

Other Medicaid members will have the COVID-19 vaccine available to them at a later date.

Please have the consent forms filled out and signed for each eligible person BEFORE your scheduled vaccine appointment.

The approach to vaccine administration will differ based on island.  This slide deck will provide more information on this vaccination effort for the CCFFHs and E-ARCHs.   Additional questions are answered in this FAQ document.

Oahu: Pharmacies will be driving directly to homes that have bed-bound residents to administer the COVID vaccine in-home, and these pharmacies include 5 Minute Pharmacy, Pharmacare, Times Pharmacy, Foodland Pharmacy, QMC POB Pharmacy & ElixRx Pharmacy.  These pharmacies will be contacting homes directly to schedule appointments.

Hawaii Island:  KTA Pharmacy will have a mix of drive thru and in-home vaccination events.  The drive thru events will occur across the island and require pre-registration.  The in-home vaccinations will be available for homes with bed-bound residents, and KTA Pharmacy will be contacting homes to schedule appointments. 

Kauai and Maui counties:  The District Health Office (DHO) will be partnering with local pharmacies to administer the vaccine.  They will have a mix of drive thru, walk in, and in-home vaccine administration.  The pharmacies will contact the homes directly to schedule appointments.  Consent forms will be provided by the DHOs.

Be sure to check your e-mail and answer your cell phones, as this is how the pharmacies will be reaching out to schedule vaccination appointments!
