Interest Form

We’re looking for current and former Med-QUEST Medicaid members, their families, and caregivers who want to make a difference by sharing their mana‘o and ‘ike (thoughts and experiences) with Med-QUEST in a feedback group.

Your input will help us understand what’s working and what needs improvement. By working together, we can learn from each other and create positive change.

No special skills required, just a willingness to share, listen, and learn together. We’re looking for individuals who are open to sharing their thoughts, collaborating with others and participating in virtual meetings. As ambassadors, you’ll help improve services and share what you’ve learned with your community.
