New Process for 1095-B Tax Forms

IMPORTANT TAX INFORMATION

Aloha,

For tax filing year 2019, the Department of Human Services Med-QUEST Division will not be mailing the annual Internal Revenue Service (IRS) Form 1095-B in January to individuals who received medical coverage in the prior year.  The Federal government has passed new regulation that has changed how individuals can receive their IRS Form 1095-B. 

You are not required to submit your IRS 1095-B when you file your taxes.  However, the Department is required to have an IRS 1095-B form available for every current and former beneficiary who had medical coverage and was enrolled in a QUEST Integration health plan for at least one day from January 1 to December 31, 2019. This form serves as proof of health care coverage during tax year 2019.
