QUEST-ACE

Eligibility Coverage Enrollment How To Apply

The QUEST-ACE (Adult Coverage Expansion) health insurance program is designed for uninsured adults with incomes up to 200 percent of the Federal Poverty Level (FPL) who meet the Medicaid asset limit and would be eligible for QUEST but are unable to enroll due to the QUEST enrollment cap. This group does not meet the financial and categorical requirements for any other Medicaid program

Eligibility

To be eligible under QUEST-ACE, the person must be an uninsured adult who meets all the requirements of the QUEST-ACE program.

A person’s countable income cannot exceed 200 percent of the Federal Poverty Level for a family of applicable size. The countable family assets cannot exceed the personal reserve standard. For a one-member family, the personal reserve standard is $2,000. For a two-member family, the personal reserve standard is$3,000.

Coverage

QUEST-ACE features the same benefit package currently provided under the QUEST-Net program:

  • 12 outpatient physician visits per year
  • 10 inpatient hospital days per year and inpatient physician visits for medically necessary medical care, surgery, psychiatric care and substance abuse treatment
  • 6 mental health visits per year
  • Prescription drugs (limited to certain antibiotic and specific contraceptives)
  • Emergency room services
  • Dental services (limited to preventive and restorative dental benefits)
  • And other services (complete list available)

Enrollment

Enrollment procedures are the same as with QUEST.

Using the link below, print out and complete the application form and mail or fax it to the nearest Med-QUEST Eligibility office listed on page 12 in the application.
