How Do I Apply?

You may apply for medical assistance for yourself and family members who live with you by filling in the Medical Assistance Application.

You may ask us to send you the form, or you may get the form at the Med-QUEST Division/Eligibility office in your area. After filling in and signing the form, you will need to return it to the eligibility office by mailing it, or taking it to the nearest office. You may also go to a public health center or hospital to obtain an application and help with the form. You will receive a phone call or letter from the eligibility office if there is anything else you need to do or send.
