3 ways to renew

  

Renew online for the fastest results

You can update your information and submit your renewal at the Med-QUEST Online Portal, even if you don't already have an online account.


Renew with a paper form

Fill out the form that came with your pink renewal notice, then mail or fax it to the Med-QUEST Division Eligibility Office nearest you.


Renew by phone

You can submit your renewal over the phone, but if we need additional evidence or documents, you'll need to send them to us by mail, email, or throught the online portal.

Renew every year to stay covered
Every year, Med-QUEST reviews your information to make sure you still qualify for health coverage.  If you don't renew by your due date, your coverage could stop, but you can reapply at any time.

We'll tell you when it's time to renew
If you need to submit a renewal, we'll tell you with a pink letter in the mail, a text message, an email, or all three to confirm.

Some people may be renewed automatically
If that's you, we'll send you a letter to confirm, text message, email, or all three to confirm.
